Tell us a little more about the point-to-point connection?

The most typical cause of these types of issues is poor network quality, 
where several packets are lost back-to-back.

Another cause of these issues can be sudden, big changes in 
network latency. Usually asterisk will automatically compensate for 
latency changes, assuming the jitter buffer mechanism is turn on (it is by 
default in HamVoIP). While asterisk is adjusting the jitter buffer, you'll 
have a few audio dropouts. But, this should rapidly settle down.
