Update, I have fixed the issue with no HDMI video out running the latest Hamvoip 1.6 firmwareon a rpi3b+.
/boot/config.txt
Change this:#initramfs initramfs-linux.img folllowingkernel
hdmi_force_hotplug=1hdmi_drive=2
To this:#initramfs initramfs-linux.img folllowingkernel
hdmi_force_hotplug=1hdmi_drive=1
Since I have no rpi4's to test this, i have no way to know if this is an issue with it as well.
However even with the latest updates using menu option #1 with my presents node's, (rpi3b+)these commands lines do not exist in /boot/config.txt.

 Yes, but it also allows HDMI hot plug. And still no HDMI video out either  way on the rpi3b+.

 I believe the RPi requires the HDMI monitor be plugged in at power up in
order for it to function.

I would plug an HDMI monitor and keyboard  into you rpi4 to confirm this in /etc/ssh. But after a install I always change this port for security reasons. Hope this helps.

Installed into Pi4.  Turn on and blinks dit-dit-dit-dah…The letter V I believe.  I try Putty and know that the ethernet IP connected to the Pi is .106 because I have another node (one of Mark’s nano-nodes).  Use port 222.  Keep getting connection timed out.  So I am stuck at the easy part…Jeez, what am I doing wrong.  I tried typing the IP into the Browser with no luck in getting a page that says TEST.
