[arm-allstar] iaxrpt howto update

When you execute the app with your account setup and enabled (checked) have
the Asterisk client opened first - selection 11 on the ADMIN menu. The you
can see what error messages are coming back. For it to register your
username and password must be correct - username and secret in the iax.conf
stanza and username and password in iaxrpt app must match.
