It appears you are correct.
It is not the slow response that threw me.
It was the poor quality of the xmit tone or perhaps the poor quality of the
9000 decode.
The COS signal is now shifting correctly in sq or ctcss mode.
The 220 radio is 4 or 5 years older and works much better than the newer
440.
Sort of echoes reports of bad radios showing up in customer comments.

Yep, that's all it is.
Squelch is tied to COS, and I think there is no pin-out for CTCSS.
So, with CTCSS enabled on the radio, COS will happen when a tone is
heard by the receiver, but it does take a little while to open. The
decoder on those radios isn't exactly fast.
The good news, at least, is that COS doesn't happen with the decoder
enabled and no tone. So it could be worse... well, except for the
deviation on the decoders is wide enough to drive a truck through.
