For the IAX port 4569 rule make sure you set the protocol to UDP, not TCP.

I can now connect from phone.

 

I'm not sure where you are trying to connect from to reach supermon. More
info needed.

I can access Supermon2 from my local PC on my LAN.  I have never configured
a 'portal' to access from the Internet. I can log in remotely to my PC so I
saw no reason to add it.
