[arm-allstar] Reverse Phone Patch

I've got a reverse autopatch for control-op / net-control users available
24/7.  There is a per-user PIN for access, when someone dials into that
line, they go to an IVR (automated attendant), as the first step. This
works very well, particularly if net-control happens not to be in range
(or has other access issues).
