Yoram,

Here is an example extenstions stanza, where the RPi node number is 1998:

[android-iaxrpt-1998]
exten => 1998,1,Answer
exten => 1998,n,Set(CALLERID(num)=0)
exten => 1998,n,Rpt,1998|Pv|${CALLERID(name)}


And, here is an example iax.conf stanza where the DVSwitch username is 
"allstar-mobile"  and the password is "XXXXXXXXX":

[allstar-mobile]
type=friend
context=android-iaxrpt-1998
host=dynamic
auth=md5
secret=XXXXXXXXX
disallow=all
allow=ulaw
allow=adpcm
transfer=no
calltokenoptional=0.0.0.0/0.0.0.0
