[arm-allstar] CTCSS on Courtesy and ID when in use

Hi
I was after a delay on TxCTCSS, if you have a courtesy tone and TxCTCSS
enable you don't here the courtesy because the TxCTCSS drops as soon as the
input CTCSS drops
Pitone V3 with the delay does exactly what I want.

Just to make you aware that the Hamvoip code can already generate DTMF -
Example - rpt cmd 1998 cop 48 1,2,3,A,4,5,6,B,7,8,9,C,*,0,#,D
Replace 1998 with your node number and the DTMF valid characters with
whatever sequence you want to send.
