Ryan,

Do you have callerid set in your android application as it looks to me 
that iax isn't picking it up.

Here's what I have  in iax.conf and extensions.conf and it works for me. 
( substituted your node number for mine. )

Only used for local wifi connection, not via Internet.

iax.conf
