[arm-allstar] remote receivers for repeater

Doug,

Do we need to add something to extensions.conf for the remote private
nodes?  Right now only references are for the local nodes.

How about username= and secret= in iax.conf?  Right now they are commented
out.

Here's what the target of the connect command is saying:
