[arm-allstar] Blacklist

allstar-public is an entirely different context, requiring no node number.
In order to use a whitelist/blacklist for WT connections, such as
Repeaterphone and DVSwitch Mobile, some additional steps need to be
taken, which, as far as I know, have never been documented anywhere.
Give me some time, and I'll come up with a writeup on this unless
someone beats me to it.
Meanwhile, if you want to block all WT connections, you can just comment
or delete the entire allstar-public context in iax.conf, or put some
custom stuff in the dialplan for iax-public in extensions.conf. Just
disabling Web Transceiver connections in the Allstarlink portal isn't
good enough. This will stop DVSM from allowing WT connections to your
node, as it honors the switch, but Repeaterphone doesn't. Your Asterisk
system will still accept unauthenticated calls on this context either
way, unless you disable the context entirely, or direct it to hang up,
or something like that.
