I found some threads describing your exact USB problem.  The suggested
cause is component failure (polymer electrolytic capacitor directly behind
the USB3 ports) on the RPi4B board, related to the power limiting for the
USB ports.  This same type failure mode has also been reported on RPi2 and
RPi3 boards.  Some have found a solution by simply removing the faulty
capacitor.  Others say replacing the capacitor fixed the issue.

If the capacitor has failed, it will probably be HOT (short circuited)! 
So, this is a possible test method.
