Lou, W2LPC here, just wanting to put in my 2¢. It sounds to me like you may have either an over voltage/transient or your COR voltage in your controller is not stable. Just a thought. Have you checked ALL your grounds, Pi pwr sup? Good luck, hope you find your bug.

Does the RED TX LED behave normally (meaning "on" all the time when TX is
asserted, "off" when TX not asserted), when unplugged from the controller?
If is does, I expect the CAT250 has an issue with its COR pin not
"floating" high and requiring active pull-down to enable.
