[arm-allstar] extensions.conf / iax.conf - IAX client disconnects immediately after connect

My client (iphone) registers fine.  I can connect but it is drops within a
few seconds.  David McGough worked with me last time I had this issue off
line, sadly I did not document the fix (so sorry).
