This is an often asked question and there is a howto for that at the

In short when multiple servers are local to a LAN you need to use local
addressing between them if your router does not support hairpin loopback.
When a node connects to another node it gets your public IP address. Since
all of your servers are the same public IP it needs to use the port to know
what server to go to on your LAN. If your router has hairpin loopback AND
you have each server on a different IAX port AND you have port forwarding
in place for each server then hairpin loopback would work assuming your
router supports it.
On the other hand if it does not support it simply inserting specific
routing information in the [nodes] section of rpt.conf for each server and
in all servers would allow any one to connect to the other.
