[arm-allstar] iaxrpt PTT keyed on start up (fix?) --was: iax rpt problem with hamvoip

If this fixes it this is already covered in our iaxrpt howto. We specify
two DIFFERENT iax.conf and extension.conf stanzas for the two methods - the
PC iaxrrpt and the phone/tablet iaxrpt. They are two different packages.
Whether the stanzas could be combined or not has not been tested but the
howto has been out there for months and apparently users who followed it
did not have the issue. See -
