[arm-allstar] iaxrpt howto update

Fred,

I am finding that for at least my Samsung android tablet it does not
matter what is in those fields but for the sake of consistency
I put my Call in CallerID and 0 in the Caller Number. I also updated this
on the iaxrpt howto, android section, with the caveat that on some Android
devices it may differ.

Again we have no control over what this app does or how it handles
parameters on different devices and there are so many out there it would be
difficult to test.

Just make sure you close the session on Android devices when you are done
because the iaxrpt app stays open and connected when you run other apps so
it would be possible to hangup something unknowingly.
