[arm-allstar] Iaxrpt

[Doug Crompton]

Callerid in iaxrpt set your node number. iaxrpt has no node number it
piggybacks off the node it is connected to. Putting a node number
(callerid) in there would be meaningless. It is set to 0 as this is the
only way to distinguish it from a real node.
