[arm-allstar] iaxrpt howto update

I also had to uncomment out the myiphone lines in extentions.conf as well
as correct the name, it was 'myphone' not 'myiphone' to match the iax.conf
information on [myiphone] settings. I wasn't able to authenticate but not
get further.

I am not sure if that was my config or the default.

(For the Android setup, not iax on Windows)
