Rick,
Yes you do need to turn off the courtesy tones or use duplex 0 as logic would say that if you have a CT it will then again key the repeater which will then again create the CT from your end and the fun begins!
If you intend to use it as a simplex node providing Allstar to a remote repeater and you have control over the repeater the best way to do it is to have the repeater output PL follow the input. Then you will ONLY hear the repeater when there is input activity and you can have a CT from Allstar. I do this on a repeater here and both the Allstar and repeater CT are audible to the repeater users but the rpeater ID does not go out over Allstar. Another option is to have two PL outputs on the repeater. An additional PL board would be needed. One PL would follow the keyed repeater and the other the input signal. The remote Allstar would use the input derived PL while user could use the repeater PL and they would hear ID's etc.

Thanks, Jeff. I'm using a Motorola Radius that has had the COS mod done to it (thanks to Kyle K0KN).
I've turned off the courtesy tones, and it seems to have fixed the problem. I still wonder if there's a better way.
