 You need to make sure the extensions and iax are setup - secret in iax.conf equals PW in iaxrpt account and that the node is setup in extensions.conf  - you have to reload both configs or restart asterisk for changes to take effect.

There is a setup howto on this on the website.

What would cause the following error when attempting to connect with the iax client? I've wrestled (unsuccessfully) with it most of the day today. Troubleshooting suggestions appreciated.
